School Forms

Parents can complete and submit most forms online prior to the beginning of the school year. Mac users must download and install Adobe Acrobat Reader in order for the back to school forms to be completed correctly.

2023-2024 Back To School Forms

  1. Download the 2023-2024 Back to School Forms ZIP file below
  2. Double click to expand the ZIP file on your computer
  3. Complete and save the PDF forms below for your child.
  4. Email these 3 PDF forms to weekdayschool@reveilleumc.org by Friday, August 11, 2023.  Be sure to list the child’s last name and “Forms” as the subject of the email.
  5. If your child has allergies or medications, the Health, Medical Authorization (MAT) & FARE Food and Allergy Forms will need to be printed, completed, and returned to the office via mail or fax (804.359.6090).
